What is on Form WH-347?

Form WH-347 has two parts. The first page is the payroll table for one week; the second page is the certification that the person submitting signs.

PageWhat it holds

Page 2, statementThe Statement of Compliance with boxes about wage compliance, apprenticeship programs and fringe benefit credits, plus a fringe table, remarks and the signature block
The statement is subject to penalties under federal law, so read it before signing.

Is WH-347 required?

No. The Department of Labor says use of the form itself is optional. What is required is a weekly payroll with the information the regulation lists and a signed Statement of Compliance with the same wording. A payroll in another format works if it contains both.

The form carries an OMB control number and an expiration date, so make sure you are using the current version from the Department's site and not a copy saved years ago.
